Brenda Grant made a “Living Will” stating that she feared degradation and dignity more than death but was kept alive after the Living Will was placed in a thick pile of medical notes.
If you make a Living Will or a Lasting Power of Attorney for Health and Welfare we recommend that:
- You tell those close to you that it exists and where it is stored.
- You provide a copy to your GP and ask them to record its existence on the FRONT PAGE of your medical notes or ensure that it is flagged on any electronic system.
- If you have a planned visit to hospital take a copy with you and make sure that the doctor or consultant in charge of your case is aware that it exists and ask them to note it on the FRONT PAGE of your medical notes or ensure that it is flagged on any electronic system.
- If you have a condition which means that you might have an unplanned visit to hospital, carry a copy of the document with you at all times.
